Reporting a workplace injury or safety incident

Applies to: US W-2 employees. Workers' compensation coverage and the injury-reporting steps below are part of being employed by Ascen. If you are an independent contractor, still report the incident to the on-site manager immediately, then email support@ascen.com — what you are covered for depends on your own agreement and insurance.

Report everything — even minor things

Please report all workplace injuries and safety incidents, even ones that seem small at the time. A minor injury can develop into something more serious, and near-misses and hazards can point to a danger that hasn't hurt anyone yet. Reporting early is always the right call, and you won't get in trouble for it.

In an emergency, get help first

If there's an immediate danger to life or health — a serious injury, a fire, a chemical exposure, or any life-threatening situation — call 911 first and get yourself and others to safety. Report the details to Ascen once everyone is safe.

Who to tell

Report to both:
1.

Your on-site manager or supervisor at the client worksite where the incident happened. They can address immediate hazards and start their own reporting.
2.

Ascen — email support@ascen.com or call your Ascen contact.

What to include in your report

The more detail you can give, the better we can help. Try to include:

What happened — a clear description of the injury, incident, hazard, or near-miss.

When — the date and time.

Where — the location or area at the worksite.

Who — anyone involved and any witnesses.

Any care received — if you were treated, the name and address of the clinic or hospital, plus any paperwork they gave you.

For a work injury, you'll be asked to complete a First Report of Injury form that captures these details. This is a routine, expected step.

What kinds of things to report

Injuries and illnesses caused by your work.

Near-misses — something that almost caused harm.

Unsafe conditions or hazards — faulty equipment, missing guards, spills, blocked exits, missing protective gear.

Safety concerns about training, procedures, or equipment.

Harassment, assault, or threats to your safety

If your safety concern involves harassment, assault, or a threat to your personal safety, tell Ascen right away so we can respond appropriately and confidentially. If you've been assaulted or threatened with a crime, we encourage you to also report it to the police or local authorities. You can reach us at HR@ascen.com.

You're protected when you report

You have the right to report injuries, hazards, and safety concerns without fear of retaliation. It is illegal for an employer to punish you for reporting a work-related injury or a safety problem, or for raising a concern in good faith. OSHA explains these protections here:
