What documentation is required for a Qualifying Life Event (QLE)?

Applies to: US W-2 employees enrolled in, or eligible for, the Ascen Workforce benefit program. Independent contractors, Canadian T4 employees, and international workers are not on these plans, so this does not apply to them.
Summary: A Qualifying Life Event (QLE) lets you change your benefits in the middle of the plan year, but only if you can show the event happened and when. The document you send is what makes the change possible — and the date on it sets both your deadline and the date your new coverage starts. Send it to support@ascen.com within 30 days of the event.
Why documentation is required
Your benefit elections are normally locked for the whole plan year. You choose your coverage when you first become eligible as a new hire, and then again each year at Annual Open Enrollment — and in between, elections generally cannot be changed. A QLE is the exception to that rule, not a general re-opening of your choices.
The reason the exception is written so narrowly is that your premiums are deducted from your pay before taxes. IRS rules for pre-tax benefit plans only allow a mid-year election change when a specific life event has actually occurred, and they require the change to be consistent with that event — getting married lets you add your spouse, but it is not an opening to switch plans for unrelated reasons. Your documentation is the evidence that the event happened, so without it there is nothing to show the exception applies and the change cannot be processed. That is why a request that arrives without documents is held rather than approved.
The date on the document is doing the most work of anything you send:
It starts your 30-day window. The clock runs from the date of the event itself, not from the day you noticed or the day you got around to sending the paperwork.
It sets your effective date. Your coverage change takes effect as of the date of the event, so the document is what determines when the new coverage — and the new deduction from your pay — actually begins.
It also protects you. A change made without support can be reversed by the plan later, after you have been treating yourself or a dependent as covered. That is the situation that leads to denied claims and premiums owed back, and sending the right document up front is what prevents it.
What to send
What counts as proof depends on the event. In each case we are looking for something official that names the people involved and shows the date:
Marriage: copy of the marriage certificate
Divorce: copy of the divorce decree or legal separation agreement
Birth of a child: copy of the birth certificate or hospital record
Adoption of a child: copy of the adoption decree or placement agreement
Death of a dependent: copy of the death certificate
Loss of other coverage: written confirmation of the loss including the end date — for example a letter from the prior carrier or employer
Gain of other coverage: written confirmation of the new coverage eligibility and its effective date
Change in employment status: documentation of the change, such as an employer letter or a pay stub reflecting it
If the official copy will take a while to arrive — a birth certificate, for example — send what you have now rather than waiting past your 30 days, and tell us the original is coming.
How to submit it
Email support@ascen.com within 30 days of the event with the change you want and your supporting document attached. Ascen handles benefit changes directly — you do not need to go through your staffing agency for this. We will confirm the change in writing, including the effective date.
