What documentation is required for a Qualifying Life Event (QLE)?

The documentation required depends on the type of qualifying event. Examples of required documentation may include:
- Marriage: Copy of marriage certificate
- Divorce: Copy of divorce decree or legal separation agreement
- Birth of a child: Copy of birth certificate or hospital record
- Adoption of a child: Copy of adoption decree or placement agreement
- Death of a dependent: Copy of death certificate
- Loss of other coverage: Written confirmation of loss of coverage, including the end date, such as a letter from the prior carrier or employer
- Gain of other coverage: Written confirmation of new coverage eligibility and effective date
- Change in employment status: Documentation of the status change, such as an employer letter or pay stub reflecting the change
