A form asks me to fill in my employer's name and address — what do I put?

Applies to: anyone working through Ascen who has been handed a form to complete themselves — a loan or mortgage application, a rental application, a benefits or immigration form, a school or childcare form — that asks for employer details. This is different from a lender or agency verifying your employment with us, which is handled a different way and is covered below.
Summary: Email support@ascen.com and ask for the employer details to use, telling us what the form is for. We will send you the exact legal name and address. Please do not copy an address off an old letter or paystub — we use more than one, and the right one depends on what the form is asking for.
Why we would rather you asked
There is more than one correct answer. We have a mailing address and a registered office, and they are different places. Some forms want the legal entity name exactly as registered; others want a corporate phone number, a company registration number or a tax ID. Picking the wrong one can get a form kicked back, which usually costs you more time than asking us did.
So rather than publishing one address here that will be wrong for half of the forms people are given, we would rather you told us which fields you need to fill in.
What to send us
Email support@ascen.com with:
Your full name as it appears on your Ascen account.
Your staffing agency and, if you know it, the client or site you work at.
What the form is for — a mortgage or loan application, a rental application, a government benefits form, and so on.
The list of fields it is asking for. Copying the labels straight off the form is ideal: employer name, address, city, state, ZIP, phone, and so on.
If the form has a deadline, say so in the email.
Who your employer actually is
If you are on a W-2 assignment, Ascen is your employer of record — your legal employer for payroll, tax and employment purposes — even though a staffing agency placed you and you do the work at a client's site. If your recruiter told you to list Ascen rather than the agency or the client, that is right.
If you are an independent contractor, you are not on Ascen payroll and "employer" is not really the right frame for your situation. Email support@ascen.com and tell us what the form asks for, and we will tell you what we can confirm.
If someone wants to verify your employment instead
If a lender, landlord or agency wants to confirm your employment or income with us rather than have you write it on a form, you do not fill anything in at all — there is a separate verification service for exactly that, and it is usually faster. See How to provide proof of employment or income.
The two get mixed up constantly, so it is worth checking which one you are actually being asked for. If the form has a section for you to complete, it is this article. If it has a section for your employer to complete or sign, it is the verification route.
Good to know
Do not guess from an old paystub, an email signature or a letterhead. Addresses we have used in the past are not always the ones to use now.
