What is Supplemental Life and AD&D coverage?

Summary: Supplemental Life is voluntary term life insurance that pays your beneficiary if you pass away while covered; AD&D adds benefits for a covered accidental death or serious injury. You choose your coverage amount.

Supplemental Life is term life insurance paid to your beneficiaries. You can elect coverage for yourself and your family:
- Employee: $10,000 increments, up to $500,000 or 5× your annual earnings.
- Spouse: $5,000 increments, up to 50% of your amount.
- Child(ren): $2,000 increments.

It includes portability, a 30-day conversion option, an accelerated death benefit, and waiver of premium. Premiums are age-banded per $1,000 of coverage. You must work at least 30 hours/week to enroll, and Evidence of Insurability is required for late entrants (see Supplemental Life Evidence of Insurability Waiver (EOI)).

AD&D (Accidental Death & Dismemberment) pays a benefit if a covered accident results in death or certain serious injuries.
