Are benefit premiums taken monthly or from each paycheck?

Last updated: November 4, 2026

Applies to: US W-2 employees enrolled in Ascen health or supplemental benefits. Ascen benefits are for US employees, so independent contractors and workers employed outside the US do not have these deductions.

Summary: From each paycheck. The prices in the benefits enrollment guide are monthly amounts, but your share is deducted from your weekly paychecks, so each deduction is only part of the monthly figure. Once you are enrolled, the Ascen portal and your pay stub show the exact amount taken each week.

Why the enrollment guide shows monthly prices

Plan prices are quoted per month, and the US Benefits Enrollment Guide lists them as monthly payroll deductions. That is the cost of the coverage for a month, not the amount that comes out of one paycheck. Your share of that monthly cost is collected through payroll every week.

So if you are comparing the guide with your pay stub, expect the weekly deduction to be noticeably smaller than the monthly price. That is normal and does not mean a deduction is missing.

What each weekly deduction pays for

Each week's deduction pays for your coverage for the following week. Some plans cover you for a whole month at a time, but their cost is still collected weekly.

Where to see your weekly amount

Weeks when you have no paycheck

If you have no pay in a week, there is no paycheck to take the premium from. On most plans, coverage ends after 5 weeks without a premium deduction. See What happens to your health coverage between assignments for what happens and how to get your elections back.

Your 401(k) is separate

A 401(k) contribution is not a benefit premium. It is a separate election you set as a percentage or a dollar amount, so it does not come from the enrollment guide prices. To check or change it, see How to Update Your 401(k) Contribution.

If the amount looks wrong

Email support@ascen.com with the pay date, the deduction line exactly as it appears on your pay stub, and the plan and coverage level you chose (for example, employee only or employee plus family). We check it against your elections and correct anything that does not match. If you see a deduction for coverage you do not think you chose, see A benefit premium was deducted but you did not enroll.
